Shawn's First Stop Health Journey

Shawn scheduled an Urgent Care appointment to refill his asthma medication. He shared he had intermittent leg pain and his Urgent Care doctor referred him to a First Stop Health Primary Care doctor.

Shawn's First Visit

Shawn scheduled a Primary Care appointment for 3 days later. During his visit, he shared he had not seen a physician in over 2 years. His doctor ordered lab work & a screening to an in-network facility.

Lab Results

He completed his lab work and screening 2 days later. His results revealed he had Stage 2 Hypertension. He was nervous about his diagnosis, so he scheduled another appointment with his First Stop Health doctor.

Follow-Up Care

During Shawn’s second visit, his doctor prescribed appropriate medication, guided him through logging blood pressure, and prescribed meal and exercise plans. He was also referred to a First Stop Health registered dietitian for further support. His dietitian helped him identify and adhere to appropriate meal plans to help lower his blood pressure. 

He met with his dietitian and doctor regularly to improve his health. 3 months later and after routine visits, his blood pressure improved but was still elevated. Shawn was committed to improving his health and continued care with First Stop Health.  His Care Team made modifications to continue lowering his blood pressure.

After 6 months, Shawn’s blood pressure was stable.

The result.

With the support of his care team, Shawn was able to stabilize his blood pressure. Care continuity ensured adherence to care plans and routine care from First Stop Health helped Shawn improve his health.
